Periodic Health Assessment (PHA)

• PHA for patients under 50 years old:
  • To schedule your annual DoD PHA, you must complete your online PHA assessment by going to https://medpros.mods.army.mil/portal, and under "Self Service" there will be a link for "Periodic Health Assessment."
  • Schedule virtual appointment through the MACH appointment line at 803-751-2273 or 833-662-7433.
  • Appointments will be canceled if Part 1 of PHA is not complete 72 hours from appointment date. 

• PHA for patients 50 years and older:
  • Will need to contact SRC at 803-751-0203 to schedule appointment in person with provider.
