Physical Exam Section (PES)/Periodic Health Assessment (PHA)


Location: Moncrief Army Health Clinic, 3rd Floor (co-located with the Soldier Readiness Center)

Hours: M, Tue, W, F: 0730-1200 & 1300-1530
Th: 0730 - 1200 (Closed in afternoon)

Services Offered through PES:

  • Periodic Health Assessment
  • Airborne, Attaché, CID, Chapter, Commissioning (OCS/WOCS/IPAP/Chaplain), Drill Sergeant, ETS, Jumpmaster, MOS, Pathfinder, Ranger, Recruiter, Retirement, Sapper, Sniper, Special Forces, Special Operations Command (SFAS, CAAS, POAS, SERE, Diver), White House
  • Automated Neuropsychological Assessment Metrics (ANAM)
  • US Army Periodic Health Assessments (PHA)
    * For Empaneled/Enrolled Beneficiaries. 
    Please complete the Part 1 Member Portion prior to contacting PES.  
    Directions to completing the online PHA questionnaire can be found at 
    * A Part 2 (provider/BH) appointment will be made once verification of Part 1 completion.
    * The Virtually Integrated Patient Readiness Remote Clinic (VIPRR) provides a decentralized and remote option for completion of PHAs.  Once Part 1 is completed, beneficiaries may contact the VIPRR Clinic at 1-844-863-3236 to complete Part 2. 
     
    * For Non-Empaneled/Enrolled Beneficiaries (e.g. outside the 50 mile radius of MAHC, non-Title 10/active Army Reservists/National Guard):
    Contact your home unit for location of servicing Logistics Health Incorporated (LHI) provider
    If eligible (on active-duty orders with eligible UIC), please contact the Virtually Integrated Patient Readiness Remote Clinic (VIPRR).
  • School / Ascension Physical Requirements
    * Contact the PES section for completion of associated diagnostic tests associated with physicals listed below.
    * Completion of Part 1 is required prior to booking a Part 2 appointment through PES. 
    * For requisite Part 1 checklists and documentation, please see the following:
    1. Documents/Checklists are to be fully completed and maintained together as a packet for successful submission.
    2. Completed Documents/Checklists may be scanned/forwarded to the following through the MHS GENESIS Patient Portal:

    * Completed Packets may be hand carried and delivered to the PES during regular business hours. Documents are as follows:
    Complete through block 29.  On block 29, if needing additional space, type "see attached" and associate responses 
    to a word document with affirmative findings in blocks 10-28. Include name, SSN and DODI on pages 2 and 3.
     
     
    Complete through 15c and include name, SSN and DODI on pages 2 and 3.
     
     
    Complete page 1 and sign.

    Cancellations: Part 2 PHA’s and Physicals can be cancelled by the appointment line or through the MHS GENESIS Patient Portal. Rescheduling Part 2’s require a message through the appointment line to PES or a direct message to PES through the patient portal to the following inbox:
