Population Health

The Population Health Nurse located in the Primary Care Clinic is main resource for health promotion and disease prevention activities. Population Health manages patient populations and ensures appropriate preventive care services are addressed. Preventive care services include health screenings, checkups, and other medical procedures to prevent illnesses, disease, or other health problems.

There are several ways to contact your healthcare team to discuss if you are due for a preventive screening or to schedule an appointment.  You may:

 
Preventive Screenings include but are not limited to:

BREAST CANCER SCREENING: A mammogram is due every 1-2 years for women ages 40 to 74 years. You do not need to see your primary care manager to schedule a screening mammogram. However, if you have noticed any changes in your breasts, you must meet with your PCM for an evaluation prior to scheduling a mammogram.

CERVICAL CANCER SCREENING: Human Papilloma Virus (HPV) is the leading cause of cervical cancer. A pap smear is due every three years for women ages 21-64 years or a pap smear + HPV test is due every five years for women 30 years and older.

COLORECTAL CANCER SCREENING: All adults, ages 45-75 years, are due for a colon cancer screening. Please speak with your PCM about screening options.

What are the most common screening options?
  • Colonoscopy: Up to every ten years based on results
  • Cologuard (multitarget stool DNA test): Stool sample every three years for average risk individuals https://www.cologuard.com/
  • FIT/FOBT (fecal occult blood test): Stool sample yearly
 
DIABETES MANAGEMENT: Diabetes A1C Screening: All persons with Diabetes Type 1 or 2, are due for a Hemoglobin A1C lab test every 3, 6, or 12 months depending on your level of control.

 

WELL CHILD VISIT:

Well child receives six well child visits during the first fifteen months of life.  The American Academy of Pediatrics (AAP): recommends the following appointment schedule: first week, first month, two months, four months, six months, nine months, twelve months, and fifteen months.
