Immunizations

Services Offered

The Immunization Clinic is operational Monday through Friday for all Soldiers, family members, retirees, their family members and children 4 years and older.

All vaccine record requests are made through medical records.

South Carolina DHEC forms are needed for children in school outside of Fort Jackson. It takes 72 hours to complete the transfer of shot records onto the DHEC form.

We encourage you to bring all Immunization records, school forms, and/or Individual Medical Reports (active duty) to reduce wait time and increase accuracy of records.

If you will be traveling overseas, you will likely need to see a provider to determine what vaccinations and/or treatments you might need, such as malaria prophylaxis. You could schedule an appointment with your PCM (Primary Care Manager) or with the TRAVEL CLINIC in Preventive Medicine. Click the hyperlink for more information.
